Fillers are one of the greatest and most dramatic in-office procedures we offer. Dr. Allan Wulc of Philadelphia is particularly skilled in facial filler techniques and has corrected bad filler injections from other providers on a routine basis. This is an effective solution for patients looking for a fuller, wrinkle-free face.
What are Fillers?
Dr. Wulc has an extensive knowledge of anatomy gained from surgical experience and artistic understanding. This allows him to use fillers to “retouch” the face, similar to a makeup artist bringing out the underlying beauty of someone’s features while hiding the flaws. The results are dramatic and immediate.
In patients who have been disappointed with previous injections, Dr. Wulc offers remedial treatment with pleasing results. Belotero® has recently been approved for the treatment of fine wrinkles. We have found it especially useful for the treatment of lip lines and hollow eyes. Fillers can be used in the lips to restore the mouth area. They can also be used around the eyelids as an alternative to eyelid surgery in efforts to conceal eyelid bags. It is minimally disruptive to tissues, which from a research point of view has many advantages.
Filler Under the Eyes
While millions of filler injections are performed every year, the majority of these injections are placed around the nasolabial fold and the lips. But fillers have a place in the restoration of volume loss throughout the face and can be used to restructure, lift and beautify.
One of the most challenging areas for injection is around the eyes. Many injectors refuse to inject near this location for fear of damaging the delicate structures of the eye because the results can be unforgiving, producing undesirable discoloration of the eyes, lumps, or swelling.
We have developed a technique of eyelid and midfacial injection which has been presented nationally. The injections are painless but can produce bruising, despite using a blunt cannula technique to minimize black and blue and hasten recovery. Please see our gallery to discover results. How We Treat Using Fillers
An Initial Consultation
During the initial consultation , Dr. Wulc will determine where the filler should be placed. Fillers are typically injected in the hollows; areas that need to have more fullness or be accentuated to improve your appearance. Once your customized treatment plan is developed and you are comfortable with the treatment options planned, Dr. Wulc will prepare the skin by applying topical numbing cream to the localized area of skin.
Identifying and Numbing Location
Depending on the location of the filler, some areas, especially in the lips, can be a sensitive area to treat. For lip fillers, your mouth is first numbed with a flavored topical anesthetic, and then with a local anesthetic that lasts 30 minutes. This keeps you comfortable during the procedure.
Inject and Sculpt
Once the area is properly numbed, the skin is cleansed of the remaining numbing cream and then injected—sculpting, restoring, enhancing and refining the area. After treatment is finished, patients are escorted to a room to relax.
It is possible bruises may appear in a few small areas. If you notice bruising, we will use light-based therapy and/or topical arnica to lessen the duration. It is also possible that the numbing medicine will make your face feel tight and make your mouth feel asymmetric. These feelings should last under an hour. Patients are told to check in the next few days for asymmetry, overcorrection, or complications. There is often more swelling the next morning than immediately following the injection, but most patients are thrilled with their appearance as soon as the anesthesia wears off.
In general, the greatest issue with fillers in our practice is that you may want more. Undercorrection—subtle enhancements and nothing obvious—is what we try to produce in most patients. If it is too subtle for you, we can always treat again. Usually, we wait two weeks before making that decision and we discuss your options together. We encourage you to call us with any concerns after the procedure.
